Putting children and young people at the heart of their own support network.
Ian Newell highlights how children can lead the conversation on their own care
A major challenge faced by community care workers looking after children and young people is keeping track of progress of a child’s case and communicating any changes to a multitude of agencies involved in their care. Small changes to a child’s mental health or risk status need to be communicated to everyone involved, much more quickly.
So Refero has linked up Connect4Care to developed a case management system for Community Mentoring and Support (CMAS) that enables children and youth service providers to manage communications across a wide range of agencies from one central platform.
